Q: Has PM/2 for DOS been updated to the 1998 revisions of the HCFA-1500 form?

Yes!
PM/2 for DOS, release 3.01 and above, can optionally print dates in the new 8-digit format now required on Medicare forms. The older format is still available for non-Medicare carriers and those which do not (yet) recognize the new format.
To implement the new format for Medicare, please do the following:
Bring up your Insurance Carrier List
(Alt+Z).
Note the "Format" column. Assign Format #2 to Medicare, leaving all other carriers at their original setting normally #0 or #1).
